Our Online Consultation is designed to help you and our team determine if you are a good candidate for Laser Vision Correction (LVC).
It was developed to evaluate your candidacy for LVC while simultaneously capturing the intake information required from all patients who visit Millennium. In combination with viewing our Advances and Benefits of LVC video, which provides a basic understanding of how these remarkable procedures work, this consultation will provide Millennium and your eye care provider with most of the information required to determine your candidacy. Since all patients are required to watch the video and complete this consultation prior to being seen, doing so in advance will significantly streamline your experience. In appreciation for doing so, you will receive a $100 courtesy if you elect to undergo treatment. This reduction can be combined with any other qualified courtesies.
Upon submission, you, our center, and if applicable, your eye care professional, will receive an email report about your candidacy. One of our coordinators will then reach out to you and if you have not already done so, set up an appointment to review this report, tour our center, examine your eyes, and take the scans required to confirm your candidacy. All responses will be held in the strictest of confidence and NO information will be given to third parties. Thank you for your time, we look forward to working with you!
